LAKERS CLASSIC REGISTRATION INFORMATION
**All teams must register at least 1 hour prior to your first game!!
Tournament Registration will be conducted at the tournament site (Middle School Fields, behind the football stadium) from 5:00-7:00 on Friday, May 16, and beginning at 7:00am on Sat., the 17th. The following documents will be needed when registering:
- Player Pass Cards
- Coaches Passes
- One copy of your State Roster
- Two copies of our Tournament Team Roster
- Travel Permit, if out of state
- Guest Player Forms
- A Signed Medical Release Form for each Player (Universal medical release forms may be used)
Medical Release Forms and Guest Player Forms can be found online at www.wiyouthsoccer.net. (Go to “Administration” then “Forms”).
The Tournament Team Roster can be downloaded from our website.
Food and Soccer Merchandise will be available at the Sports complex.
Please Note: There are NO dogs allowed on the Tournament Grounds.
